LANCASTER
1848 Charter Lane (Greenfield Corporate Center)
Lancaster

Directions to Greenfield:

From Route 30 East/West:

• Take Greenfield Road exit
• Turn right at traffic light onto Greenfield Road (landmark—Hampton Inn on right)
• Turn right onto William Penn Way
• Take first right onto Charter Lane
• LMA is the last building on the left at 1848 Charter Lane (across from Magic Years Daycare Center)
• The office is located on the lower level of the building.

WILLOW VALLEY

Directions to Willow Valley: 

From Lancaster City: 

• Follow South Prince Street (Rt. 272 south) to Willow Valley Complex. 

• At traffic light, there is a McDonald's on the left.  Turn left into the complex (shopping center). 

• Looking at the major store, Darrenkamp's Grocery (Sure Fine) you want to go around the right side of the building. 

• On the right side of the building, you will see a handicap entrance which takes you to the second floor of the complex. 

• Go down the hall which has the hair salon, until you reach the reception area for Life Management Associates (LMA).

• From areas south of Willow Street:  Follow Rt. 272 or Rt. 222 north to the Willow Valley Complex.  At the McDonalds, turn right into the shopping center.

• Please Note:  If you arrive before 4:30PM, there is a receptionist to greet clients.  If after 4:30PM, please have a seat in the waiting room and the therapist will be out shortly.

EPHRATA
Lehman Insurance Agency - 1020 Sharp Avenue (Rt. 322)
Ephrata

Directions to Ephrata:  

From Lancaster: 

• Follow Rt. 272 North (or from Denver/Stevens, follow Rt. 272 South) to Rt. 322, Exit West. 

• Follow Rt. 322 (Main Street) towards Lincoln. 

• Go two miles and turn left at Sharp Avenue. 

• The office is located on the 2nd floor of the Lehman Insurance Agency building which will be to your left. 

• Enter through the main entrance and go to the 2nd floor.  Have a seat in the waiting room. 

• There is no receptionist at the Ephrata office of Life Management Associates.

• You may also reach Rt. 322 from Rt. 222 (going north from Lancaster, or south from Reading).  Exit west on Rt. 322, go through downtown Ephrata and through intersection with Rt. 272.  Follow instructions above.

SILVER SPRING

Directions to Silver Spring: 

From Route 30 East / West

• Take Rt. 30 (East/West) to Mountville Exit. 

• Turn onto Stony Battery Road (heading towards Landisville). 

• Go approximately one mile to traffic light (large Church on right). 

• Turn left at traffic light onto Marietta Avenue. 

• Go approximately 2-3 miles. 

• Turn right into Silver Spring Shopping Plaza. 

• We are the last office building on your right (before residential development). 

• Office is located on the 2nd floor, Suite D.  There is no receptionist at the Silver Spring office. 

• There should be a clipboard on the table. 

• Please complete the intake sheet while you wait.
